Or, if you suddenly notice the little guy on the right showing up at your gigs, you might really want to get those brakes on your car checked. 
Either way, you know it can’t be a good sign when you’re either playing with a clown, or seeing a sax-playing skeleton. It’s either a) Time to get a better gig; or b) Time to get your meds checked. Maybe both!
